孤独在诱发抑郁的同时,促进饮酒行为

长期被隔离比工作压力更容易引起抑郁。COVID-19新冠疫情大流行期间引起的社会隔离显著增加抑郁和焦虑的患病率。抑郁症患者自杀风险是正常人的20倍到40倍。为逃避悲伤和忧郁情绪,抑郁症患者容易过度饮酒。临床报告表明大约 40.3% 的重度抑郁症患者患有酒精使用障碍,反过来,酒精使用又会增加抑郁症患者自杀风险。
更可怕的是,饮酒行为被认为是在治疗抑郁症过程中的一大“拦路虎”:导致对抗抑郁药物的反应降低,这一比例高达 70%。此外,长期大量饮酒逐渐钝化奖赏系统功能,导致饮酒频率和饮酒量增加,成为酒精使用障碍患者,对酒精产生依赖。 近日,韩国大田大学的研究团队在 Science 子刊 Science Advances 上发表了题为:Social isolation-related depression accelerates ethanol intake via microglia-derived neuroinflammation的研究论文。  
研究人员利用社交隔离模型发现小鼠在单独饲养第8天后酒精饮用量就开始增加,在条件位置偏好实验中也进一步证实了社交隔离促进酒精寻求行为。社交隔离后小鼠的糖水消耗降低、强迫游泳不动时间增加等抑郁样行为。此外,社交隔离也诱导小鼠出现焦虑样行为,这种焦虑在摄入酒精后更加严重。
此外,社交隔离促进大脑奖赏系统腹侧被盖区多巴胺信号,进而引起酒精使用增加。这就表明酒精使用能够加重社交隔离引起大脑的不良反应。研究人员发现社交隔离模型小鼠中缝背核的突触蛋白PSD95 和synaptophysin表达降低,突触吞噬相关的补体C1q和C3表达增加,促炎症因子TNF-α水平升高,抗炎症因子IL-10水平降低。腹腔注射小胶质细胞抑制剂米诺环素后能够降低社交隔离小鼠的小胶质细胞的激活,补体C1q表达水平也恢复正常水平,并能缓解抑郁样症状。
总的来说,本文发现社交隔离引起的抑郁和焦虑样行为,在酒精使用后这些负向情绪被进一步加重。
